Reimbursement Request Form

Use this form to request reimbursement from pre-approved expenses for fundraisers and events.  Please contact either the Treasurer or Co-Presidents prior to purchase/spending for approval of the expense.  Also please submit all receipts pertaining to the expense(s) attached to the form.

Drum Majors and Color Guard Captains:  The MPA will reimburse $250 of the fee for the Drum Major Academy (DMA) training for one time only.  Please use this reimbursement request form with the receipt and proof of attendance after participation in the DMA program.
